Ban Appeal Format

In the event that you do get banned from any of the Vivacity servers, we simply ask that you post a ban appeal in this format, posting in a different format will automatically cause denial of your appeal and you will have to restart the appeal process. The format is:

In Game Name:
The Name That You Use IN THE SERVICE YOU GOT BANNED ON.
Have you changed your Minecraft username since being banned? If so, what is your previous name:
Only applies if you were banned on a Minecraft server.
Which of Vivacity's services are you banned on:
(Discord, Vivacity Minecraft, ETC)
Reason of Ban:
Why Were You Banned (Because a Admin was mean, is not a good answer)
Rule Violated:
What Rule did you violate in the server (Refer to the rules page and post the exact rule that you violated)
Please explain in your own words what the rule(s) means:
Explain what the rule means
Reason you feel you think you should be un-banned:
Explain why you should be unbanned
